Common. A person or thing that takes the place of another. Highly interchangeable, especially when referring to the people or objects themselves that are used in place of others, or the act of putting them in place. 'We need replacements for the broken parts' vs. 'we need substitutions for the broken parts'.

Common. The act of giving one thing and receiving another in return. Not always interchangeable. 'Exchanges' often implies a reciprocal act or trade, whereas 'substitution' is one-sided replacement. 'A prisoner exchange' works, but not 'a prisoner substitution'.
